ANSWERS TO FREQUENTLY ASKED QUESTIONS WHAT KIND OF FACILITY ARE YOU? We are a Social Model Adult Day Care Program specializing in care for seniors with early stage dementia/Alzheimer’s, Parkinson’s or Mild Cognitive Impairment, who need a safe/secure environment, social interaction and mental/physical stimulation.
WHAT KINDS OF ACTIVITIES DO YOU DO? Our schedule consists of different activities every 30 to 45 minutes to alternately stimulate the body, mind & spirit. Examples: painting, music, brain games, chair exercises, arts & crafts, strolls outside, creative writing, etc.
WHAT ARE YOUR REQUIREMENTS/CRITERIA FOR ENROLLMENT INTO THE PROGRAM? We are a Social Model, rather than a Medical Model program. Our clients must be “weight bearing,” and have the ability to walk, eat and use the restrooms independently or with minimal assistance. Walkers and wheelchairs are permissible. Our clients should also be able to understand simple directions and follow them. As we are not a Medical Model program, we do not provide ongoing Care Management. We do conduct regular evaluations and will communicate regularly to the families if there are changes in their loved one, and we are happy to refer to our community partners who provide the Medical Model.
DOES ANYONE IN YOUR PROGRAM HAVE ALZHEIMER’S OR DEMENTIA? Yes, our program benefits those who have the beginning to mid stages of Alzheimer’s or Dementia, but not everyone who attends our program has Alzheimer’s or Dementia.

ARE THERE CERTAIN DAYS MY LOVED ONE WILL NEED TO COME OR CAN I JUST DROP HIM/HER OFF ANYTIME HE/SHE FEELS LIKE COMING? We require a 6 day a month minimum attendance and we prefer a regular set schedule. Client schedules are requested by the 20th of the month for the following month. For example, on September 20th, the schedule for October is due to the Director. We ask this so that we are properly staffed per licensing requirements.

WHAT DO I NEED TO DO TO GET MY LOVED ONE STARTED IN YOUR PROGRAM? We require a two-day preliminary evaluation before enrollment into the program. This gives them and our staff an opportunity to find out if the program is right for them. The hours for the two-day assessment are from 9:30 a.m. to 2:30 p.m. and the fee is $170.00. After the two evaluation days, if it is decided that our program will be a good fit for them, we can proceed with the enrollment process. You can let us know what days your loved one will be attending and we will be sending some paperwork home with you to fill out and bring back on their first day of attendance. A physician’s report and a recent TB test are required for enrollment into our program.
DO THE TWO EVALUATION DAYS HAVE TO BE CONSECUTIVE? No. They can be any two days that will work best for you.
We are pleased to welcome you and your loved one to our family!
